Affirmation Statement

Information in the Recorder's office is public information and Social Security numbers are no longer allowed on documents submitted for recording per IndianaCode 36-2-11-15. All documents that are notarized in the State of Indiana must have the "affirmation statement" included immediately before or after the "prepared by" statement. Documents which are acknowledged outside of Indiana and which do not have the affirmation statement, still may not contain any Social Security numbers. The only exceptions to this rule are:

  1. an instrument executed before July 1, 1959 or recorded before July 25, 1967.
  2. a judgment, order, or writ of a court
  3. a will or death certificate
  4. an instrument executed or acknowledged outside of Indiana
  5. a federal lien on real property or a federal tax lien on personal property

An instrument complies with the law if it contains the following wording:

" I affirm, under the penalties for perjury, that I have taken reasonable care to redact each Social Security number in this document, unless required by law (Printed Name)."
